I would like to add though, that this is the situation for all pharmaceuticals, not just pain meds. The treatment you often get for any condition follows this formula: what sales pitch your doctor fell the most for + whether your insurer is contracted with that pharma co. (the latter can get complicated as there may be a big third party inbetween there, i.e. Optum Rx.)
If your doc is getting up there in years or just doesn't care about putting in more hours and therefore doesn't keep up with the latest marketing (goes for procedures as well, I might add), you could get old style med'cine. Which has its own downsides, of course. It could also be very money-driven, just in a different way.
